The family or person designated below declares to register as a member of the EFY

Annual fee CHF 40.-       From September 1st CHF 20.-

Any family or person who has submitted this form is considered a member of the EFY 
upon payment of the first contribution.
The contribution is payable at the store checkout or on our IBAN CH26 0900 0000 1001 6837 7, remembering to mention your first and last name.
